Submit Documents

• Click [ Add Document ] below to select the files. These can be electronically filled out PDF documents, pictures of the original documents taken on your phone, or scanned images.
• Only the following file types are accepted: *.pdf, *.png, and *.jpg / *.jpeg
• Before any documents leave your computer or device, a preview of the files you have selected will appear for you to make sure that they are the correct files.
• Double check that the email address entered is correct.
• If everything looks good, click [ Submit ] below to send the documents.
